I, {name}, acknowledge and understand that participation in Brazilian Jiu-Jitsu (BJJ), Mixed Martial Arts (MMA), Wrestling, and related physical activities, whether on or off the gym premises, inherently involves risk of injury, including but not limited to sprains, fractures, concussions, bruises, muscle tears, and other injuries, some of which could be serious or life-threatening. By signing this waiver, I voluntarily assume all risks associated with participation in these activities.
I agree to comply with all safety instructions and guidelines provided by the instructors and staff at Folkestone Brazilian Jiu Jitsu and agree to use all equipment properly.
I release Folkestone Brazilian Jiu Jitsu, its employees, agents, affiliates, and any other individuals or organizations involved in the gym's operation from any liability or responsibility for injuries or accidents that may occur during my participation in training or classes.
I certify that I am physically fit and capable of participating in Brazilian Jiu-Jitsu, MMA, Wrestling, and related physical activities. I agree to inform the gym staff of any known medical conditions or limitations that may affect my ability to safely participate.
In case of an emergency, I authorize Folkestone Brazilian Jiu Jitsu staff to seek medical treatment for me, including but not limited to first aid, transportation to a medical facility, or any necessary medical treatment. I understand that I am responsible for all medical expenses incurred as a result of any injuries or accidents during training.
I grant permission to Folkestone Brazilian Jiu Jitsu to use any photographs, video recordings, or audio recordings of me taken during training or events for promotional, advertising, or other legitimate purposes. I understand that I will not be compensated for the use of these materials.
I agree to indemnify and hold harmless Folkestone Brazilian Jiu Jitsu, its staff, agents, and affiliates, from any and all claims, demands, or causes of action that may arise from my participation in activities, including any injuries, damages, or losses incurred during training.
I understand that as a member of Folkestone Brazilian Jiu Jitsu, I am required to follow all gym rules, policies, and guidelines as set forth by the instructors and gym management. Any violation of these rules may result in termination of my membership without refund.
By signing this waiver, I confirm that I have voluntarily assumed all risks associated with participation, I release Folkestone Brazilian Jiu Jitsu from any liability, and I authorize the use of my image for promotional purposes. I also certify that all information provided is accurate and complete.
By signing below, I acknowledge that I have read and fully understand this waiver, and I am voluntarily agreeing to its terms. I further affirm that I have not been coerced into signing this waiver and that I am of sound mind and body to make this decision.
I acknowledge that I have been given an opportunity to ask questions about this waiver and have received satisfactory answers. I confirm that I am signing this waiver of my own free will and understand that this agreement is binding.
